General
The Enrollment Agreement is between Dolce Music Institute and the students/parents. The Enrollment is for the 2020‐2021 school year, from 8/16/2020 to 7/24/2021. If you have plans for a vacation, please notify us, we will arrange makeup lessons accordingly. Early termination is cancellation. Parents and students can only choose a preferred teacher based on the teacher's availability. If there is no preference, Dolce Music Institute will recommend teachers for the student. Lessons will take place at Dolce Music Institute. If parents request at‐home lessons, there will be an additional trip fee of $5 every lesson, and the payment can only be paid via credit card.
Payment
The number of lessons for the school year is 44. (Please refer to the calendar) There is a $40.00 registration fee when submitting the form. Starting this school year, all students/parents are required to leave their credit card information when registering. Dolce Music Institute will use the information to make auto‐payments on the 15th of each month. Alternative payment options include check, cash, and online payment. Cash and checks must be received no later than the 10th of each month. Online payment options include A) Zelle, 469-777-1363 B) Venmo, dolcemusic. Please make checks payable to James Chen.

The Student/Parent has to fill out the credit card automatic payment form when submitting the registration form. Each billing cycle includes four lessons. During the pre‐registration period or the first month of every new semester, parents can pay the full amount for the semester for a 2% discount applied to the overall tuition of the semester. This payment can ONLY be paid with cash or checks. SCHOOL YEAR PAYMENT. During the pre‐registration period or the first month of the school year, parents can pay the full amount for the school year for a 4% discount applied to the overall tuition of the school year. This payment can ONLY be paid with cash or checks.
By leaving credit card information when registering, I authorize Dolce Music Institute to initiate automatic payments on the 15th of each month from my account. I understand Square will send me an email when the charge happens. I understand Dolce Music Institute will charge on the credit card if I don’t make the payment by the 14th of each month.

Absence
If a teacher is absent, Dolce Music Institute will arrange a highly-competent substitute for the student. The lesson on the schedule will remain. When a student needs to be absent, DMI must be notified at least 24 hours in advance. We will then arrange a teacher or a substitute to schedule a make‐up lesson. (Please call us in a case of emergency)There are only three (3) available make‐up lessons allowed in a semester. If more absences are needed after all three make-up lessons, lesson payments will still be made, and no make-up lessons will be arranged/scheduled.After requesting a makeup lesson, there is not a second make-up lesson regarding the same absence if a student misses a makeup lesson.
Transfer, Cancellation or Emergency
In order to maintain the high quality and standard for the teaching, if the students and parents wish to discontinue lessons, they are required to fill out the TCL Form and submit to Dolce Music Institute ONE MONTH (30 days) prior. In the case of cancellation, there will be a withdrawal fee of $30.00. The rest of the monthly, semester, and school year payment will then be refunded via transfer or check. In terms of emergency, when the city issues any order to forbid the students/teachers to take lessons in person, we will move the lessons online. FaceTime, Skype or Zoom.
